Examples include chronic pain, cancer, post traumatic stress disorder, migraines, osteoarthritis, rheumatoid arthritis, neuropathy, multiple sclerosis, Crohn’s disease, ulcerative colitis, seizures, and severe fibromyalgia. This is a summary, not the complete list.

Symptoms and qualifying diagnoses are different
Anxiety, depression, nausea, and muscle spasms should not be presented as a general list of standalone qualifying diagnoses. The clinician evaluates your actual diagnosis against the current program requirements. A symptom or an appointment alone does not guarantee eligibility.
When your diagnosis is unclear
Bring the records requested for your evaluation. Our administrative team can explain scheduling and document preparation; the clinician addresses diagnosis and certification questions. Does anxiety by itself appear on the IDPH list?
Anxiety is not listed as a standalone condition on the IDPH list reviewed for this page. PTSD is a separate listed diagnosis. Ask the clinician about your actual diagnosis.
Does a listed condition guarantee a card?
No. Certification and the state application decision are separate steps. Program requirements and your individual circumstances still matter.
